3. Confidentiality
PRIVACY BREACH

Problem: On Wednesday July 28, 2010, BPDFamily sent out a routine e-mail newsletter to a list of inactive members. New software was utilized. While the outgoing e-mail address were encoded and private, anyone who responded to the newsletter had their e-mail address disclosed to all of the other subscribers on the mailing list. We realized quickly (73 min ) that there was a problem and immediately shut down the system to prevent any further issues. Unfortunately, the eleven members who responded to our message had their email address disclosed. All the subscribers on the list received copies of 14 e-mail responses intended for BPDFamily. Corrective Action: BPDFamily is committed to protecting our members privacy and sense of safety. We took immediate action so that no such errors will occur in the future. The software and database were destroyed immediately following the incident. The protocol for testing futures mailing lists and newsletter software has been revised.

Notification: All recipients received a follow-up e-mail 133 minutes after the newsletter mailing explaining the situation. Apology letters were later sent to the eleven members that had their e-mail addresses disclosed.
